Hello, PATH Participants!

Providing Access and Transforming Health (PATH) is a five-year initiative through the California Department of Health Care Services to build up the capacity and infrastructure of on-the-ground partners, such as community-based organizations, public hospitals, county agencies, tribes, and others, to successfully participate in the Medi-Cal delivery system as California widely implements Enhanced Care Management and Community Supports and Justice Involved services under CalAIM.

Upcoming In-Person PATH CPI Convenings

In place of a virtual August joint PATH CPI convening, PHIL is excited to announce that we will host three in-person convenings for organizations and providers across the Northwest and Southwest PATH CPI regions. You are welcome to sign up to attend as many of the in-person convenings as you would like. They are open to all organizations that work with providers serving the listed counties; therefore, you do not need to be physically located in one of these counties to attend. If your organization interacts with or refers clients to providers in the listed counties, you are welcome to join the conversation.
